Summary: The education of surgical residents and fellows is a complex, dynamic process and the pace of change has rapidly accelerated in recent years. Vascular surgeons across all practice models and settings are engaged in training the future workforce and must be aware of innovations, particularly new approaches to competency assessment and certification. Entrustable Professional Activities (EPAs) are the most disruptive innovation in assessment in more than a decade, and will dramatically alter the way we train the vascular surgeons of the future. Furthermore, EPAs are much more than an assessment system. They define the foundation of the vascular surgery specialty and the training benchmarks for performance to ensure graduates are prepared to provide safe, cost-effective, high quality patient care in the unsupervised setting. Implementation of the EPA system will have far-reaching impacts, including the downstream transition from training to unsupervised practice for both the academic and community settings. The American Board of Surgery has already implemented EPAs in General Surgery and Vascular Surgery is poised to be the next surgical discipline to embrace this innovative model, moving toward competency-based, rather than merely time-based training. As the VSB and APDVS are committed to implementing EPAs in academic year 2024-25, there is an urgent need to train vascular surgeons on their purpose and use to ensure valid assessment of trainee competence.
